Registration is now available online! Please follow this link to reach our Application for Registration Once you have completed the online application, you will be asked to upload the documentation needed to register with the CDHNS. 

As you proceed through the online application, you will be provided with an application checklist to assist you in submitting all the required documents. For ease of reference, this application checklist is also provided here. Please select the year relevant, to your Application for Registration to view the applicable checklist November 1, 2017 - October 31, 2018 or November 1, 2018 - October 31, 2019. For individuals requesting reinstatement or  re-licensing after your licence has expired, please contact CDHNS directly to obtain your customized checklist.

Application files are confidential and information can only be provided directly to the applicant. Applicants must provide the CDHNS written authorization before information will be released to a third party including employers, spouse, or agency.

Letters of Good Standing from the CDHNS are available through your member profile. Members may follow this link to log in to their membership profile and submit a request for a Letter of Good Standing to be sent. 

Translation of Documents to be submitted to the CDHNS as part of the Application Process

Documents that are not in English or French must be translated. Applicants are responsible for having their documents translated by a certified translator and providing a certified official translation to CDHNS of the requested documents. Costs will vary depending on the volume of the text. A certified official translation means that a professional (certified) translator takes your document in its original language and translates it into English or French. The professional then places his or her stamp, seal, or special paper on a cover letter attached to the documents themselves or to an electronic version (disk or thumb drive) indicating that it is a true translation of the original language document. If you had your documents translated for another organization e.g., NDHCB, we will accept a copy of previously translated documents that meet the criteria stated above, if sent directly from that organization.

Payment Methods

When you submit any application form, (e.g., application for registration, renewal, or transfer from non-practising to practising), you will be informed about the accepted methods of payment.

For all applications, we now accept credit card payment through Plastiq.

Using Plastiq, a third-party service provider, you can pay with MasterCard, Visa or American Express:

  • Go to https://cdhns.plastiq.com and register with Plastiq to create an account.
  • Tell Plastiq the amount you want to pay and when you want to send it.

To ensure you are appropriately credited, you will be required to include the following information:

  • The first name and last name of the individual who submitted the application form.
    • This must match the name on the initial application (if this payment is for a first time applicant) or the name of the CDHNS member as it is entered in the CDHNS database.
  • The Invoice number that will be generated following submission of your application.

Please note that payments may take 3-5 business days to process and that a Plastiq service fee will be applied (2.5% for credit cards, 1% for debit cards).
