The licence year for CDHNS runs from November 1 to October 31.

New Applicants

Registration is available online. Please follow this link to reach our Application for Registration. Once you have completed the online application, you will be asked to upload the documentation needed to register with the CDHNS. 

As you proceed through the online application, you will be provided with an application checklist to assist you in submitting all the required documents. For ease of reference, the application checklist is also provided here. Registration and Licensing Application Checklist for November 1, 2020 to October 31, 2021.

Please note: During the months of Sept/Oct, we ask you to carefully consider if you plan on applying for the licensing year that ends on October 31, OR for the upcoming renewal year that begins on Nov 1. You will be asked by staff to clarify your intent after an application is received. The full licensing fee is due regardless of when you are issued a practising licence during the year. 


The attached document provides you with a listing of potential application, registration, and licensing fees.


For individuals requesting reinstatement or  re-licensing after your licence has expired, do not submit an Application for Registration. Log onto your CDHNS Member Profile using this link and submit the correct form e.g., application for reinstatement. Please contact CDHNS directly if you need your user name and password. You will also be provided with a customized checklist after you submit the correct application form.

Application files are confidential and information can only be provided directly to the applicant. Applicants must provide the CDHNS written authorization before information will be released to a third party including employers, spouse, or agency.

Verification of Registration/Licensing (often called "Letters of Good Standing") from the CDHNS can be requested through your Member Profile. Members may follow this link to log in to your member profile and submit a request for a Verification of Registration/Licensing to be sent. 

Translation of Documents to be submitted to the CDHNS as part of the Application Process

Documents that are not in English or French must be translated. Applicants are responsible for having their documents translated by a certified translator and providing a certified official translation to CDHNS of the requested documents. Costs will vary depending on the volume of the text. A certified official translation means that a professional (certified) translator takes your document in its original language and translates it into English or French. The professional then places his or her stamp, seal, or special paper on a cover letter attached to the documents themselves or to an electronic version (disk or thumb drive) indicating that it is a true translation of the original language document. If you had your documents translated for another organization e.g., NDHCB, we will accept a copy of previously translated documents that meet the criteria stated above, if sent directly from that organization.

Payment Methods

When you submit any application form, (e.g., application for registration, renewal, or transfer from non-practising to practising), you will be informed about the accepted methods of payment.

For all applications, we now accept credit card payment through Plastiq.

Using Plastiq, a third-party service provider, you can pay with MasterCard, Visa or American Express:

  • Go to https://cdhns.plastiq.com and register with Plastiq to create an account.
  • Tell Plastiq the amount you want to pay and when you want to send it.

To ensure you are appropriately credited, you will be required to include the following information:

  • The first name and last name of the individual who submitted the application form.
    • This must match the name on the initial application (if this payment is for a first time applicant) or the name of the CDHNS member as it is entered in the CDHNS database.
  • The Invoice number that will be generated following submission of your application.

Please note that payments may take 3-5 business days to process and that a Plastiq service fee will be applied (2.5% for credit cards, 1% for debit cards).
